Service Animal Review Form

We are committed to accommodating guests with service animals in accordance with ADA guidelines.

For the safety and flow of our cooking classes, all service animal requests must be submitted in advance and reviewed prior to arrival.

• Pets and emotional support animals are not permitted.

• Only trained service animals are allowed.

• Submitting this form does not guarantee immediate review, especially for same-day requests.

• Guests may not arrive on-site with a service animal unless the request has been reviewed in advance.
